Job summary

Medix is seeking a Pharmacist to provide telephonic medication consultations, conduct annual medication reviews, and assess pharmacotherapy for adult and geriatric patients. Qualifications

Responsibilities

• Conduct telephonic medication consultations meeting production and quality metrics.
• Perform annual medication reviews focusing on patient assessment and safe use of medications.
• Assess pharmacotherapy in claims, PMH, and disease states.
• Identify adherence barriers and develop plans to improve adherence.
• Deliver evidence-based therapeutic recommendations to providers.
• Provide patient-centered, clinically collaborative consultations.
• Create post-consultation action items for patients/prescribers.
• Document consultations in internal database.
• Participate in staff meetings and cross-department initiatives.
• Identify system improvements for operational efficiency.
• Maintain knowledge on chronic diseases in adults and geriatrics.

Job Summary

Our client is seeking a dedicated Pharmacist to conduct telephonic medication consultations, focusing on annual medication reviews, patient assessment and counseling, and the identification of potential drug interactions. This role involves pharmacotherapy assessments, addressing medication adherence barriers, providing evidence-based therapeutic recommendations, and documenting clinical consultations. The ideal candidate will maintain a high level of clinical knowledge in chronic disease states for adult and geriatric patient populations.

Key Responsibilities

• Conduct telephonic medication consultations that meet/exceed production & quality metrics.
• Complete annual medication review of prescription and non-prescription medications with emphasis on patient assessment and counseling, safe and effective use of medications, identification of potential drug interactions, and other related activities.
• Complete pharmacotherapy assessment of patients' claims, past medical history, and disease states.
• Assess patients for medication adherence barriers and develop a plan to maximize patients' medication success through improving or maintaining adherence to medications.
• Deliver high quality evidence-based therapeutic recommendations to providers.
• Deliver a patient-centered, clinically collaborative medication consultation.
• Develop a post-consultation list of action items for the patient and/or prescriber.
• Document clinical consultations into internal data base.
• Participate in and contribute to staff meetings and other staff development opportunities and interdepartmental work groups.
• Identify areas of improvement where systems modifications could result in operational advancements and efficiencies.
• Maintain a high level of clinical knowledge of chronic disease states and changes in pharmacy practice in adult and geriatric patient populations to serve as an authoritative information source on drugs and their utilization in therapy.

Qualifications

• Experience with Medicare STARs & HEDIS Measures.

• Ability to manage multiple complex, concurrent projects.
• Comfortable working in a rapidly changing and highly matrixed environment.

Experience

• 3+ years Clinical Pharmacy Practice experience.

Skills

• Excellent written and verbal communication and relationship building skills.
• Planning and organizational skills to demonstrate leadership and initiative.
• Strong interpersonal skills.
• Ability to work independently, as well as a member of a team.
• Proficient with Microsoft Word, Excel, Outlook, PowerPoint.

Education

• Doctor of Pharmacy degree or Equivalent Experience.
• Post Graduate Training (Pharmacy Residency and/or Fellowship) preferred.
• Board certification by the Board of Pharmacy Specialties (BPS) preferred.
• Certified Geriatric Pharmacist preferred.
• MBA/MPH Degree preferred.

Additional Requirements

• Must hold active, unrestricted pharmacy license in state of primary residence.
• Bilingual preferred, with preference given to Spanish (Vietnamese, Cantonese, Mandarin, & Russian also considered).
• Schedule: 9:00 AM - 6:00 PM Central, Monday-Friday.
