Job summary

Southern Ohio Medical Center in Portsmouth, OH seeks a Social Work Student Discharge Planner to support patients and families with discharge planning and referrals based on age and needs. Under supervision, you will perform duties that ensure safe transitions from hospital to community resources.

Key responsibilities include coordinating post-discharge services, completing assessments, and liaising with physicians, organizations, and outside agencies.

Qualifications

• High School Diploma or equivalent required.
• Enrolled in MSW program or completed MSW degree.
• BLS within 90 days of hire.

Responsibilities

• Arranges continuing care post discharge with community resources.
• Completes assessments and follow-ups for patients/families.
• Serves as a liaison between patients, physicians, organizations and outside agencies.
• Responds to high-risk screenings and physicians' consults per criteria.
• Maintains accurate records in patient charts and department statistics.
• Participates in team and department meetings and hospital committees.
• Participates in Quality Improvement programs.
• Maintains skills through continuing education and current resources.
• Coordinates discharge planning by meeting with patients and families and collaborating with providers.
• Acts as liaison among patients, families, physicians and external agencies.
