Job summary

Prominence Health, a value-based care organization under Universal Health Services, seeks a Social Worker to coordinate transitions from hospital to home and connect members with essential resources. You will work with a multidisciplinary team to improve outcomes and reduce readmissions in a hybrid Reno, NV setting.

Under supervision of the Transition of Care Clinical Program Manager, you will engage with hospitalized high-risk members, educate families, and support post-discharge planning to

Qualifications

• Master's Degree in Social Work required.
• Active LCSW licensure in NV, TX, or FL is required or able to obtain in those states.
• Experience with Medicare/Medicaid populations is required.
• Ability to communicate in multiple languages per licensure and market needs.

Responsibilities

• Collaborate with interdisciplinary care teams to transition members from acute/post-acute care to home.
• Engage outpatient members to access resources and avoid poor health outcomes and readmissions.
• Conduct assessments and educate members and families on discharge needs and resources.
• Support development of social policy and programs to address community health issues.
