Qualifications

• MSW degree required.
• Minimum 1 year hospital social work experience; outpatient preferred.

Responsibilities

• Collaborate with RN case managers, nurses, physicians, and staff to identify patient needs.
• Identify and address barriers to discharge across financial, emotional, psychosocial, medical, and spiritual domains.
• Maintain knowledge of Medicare, Medicaid, and private payer regulations and managed-care concepts.
• Perform thorough assessments and develop discharge plans for high-risk patients.
• Coordinate meetings between care team, patients, families, and caregivers; support complex decision making.
• Provide psychosocial support, counseling, advocacy, and referrals for resources as needed.
• Offer emotional support to patients and families during end-of-life or crisis situations.
