Island Home Care, part of the UnitedHealth Group family, seeks a Per Diem Social Worker to provide medical social services under physician direction and collaborate with an Interdisciplinary Group to address social and emotional factors affecting patient health.

The role involves building an integrated Plan of Care, guiding the care team, and coordinating bereavement services when needed. Requires MSW, active SW licensure, CPR, and a valid driver’s license.

Qualifikationen

• Master's Degree from a Council on Social Work Education accredited program.
• Current CPR certification.
• Licensed Social Worker in the state of residence.
• Valid driver's license, vehicle insurance, and access to a dependable vehicle or public transportation.
• 1+ years of social work experience in a health care setting.
• Bereavement coordination experience (preferred).
• Experience with establishing a plan of care for bereavement needs (preferred).

Aufgaben

• Assist patients, family members, physician and health care team to understand social and emotional factors related to health status.
• Contribute to development of a comprehensive, integrated Plan of Care for patients daily.
• Instruct health care team on community resources to assist patients as needed.
• Act as Bereavement Coordinator and supervise bereavement services, with a care plan and delineated services.
