Job summary

Caretenders, a part of LHC Group and UnitedHealth Group, seeks a Medical Social Worker to provide medical social services under a physician and Interdisciplinary Group, addressing social and emotional factors affecting health. Responsibilities include helping the team develop a comprehensive Plan of Care, coordinating bereavement services when needed, and guiding referrals to community resources.

Qualifications include a MSW, current CPR, state licensing, valid driver’s license, and 1+ year

Qualifications

• Master's Degree from a school of Social Work accredited by the Council of Social Work.
• Licensed Social Worker in the state of residence.
• Current CPR certification.

Responsibilities

• Assists patients and family to understand social factors affecting health.
• Contributes to development of a comprehensive Plan of Care.
• Instructs on community resources available to assist patients.
• Bereavement coordination when needed and plans of care addressing bereavement needs.

Education
