Job summary

ThedaCare Regional Medical Center - Appleton, Wisconsin is seeking a qualified MSW to provide quality psychosocial care to patients and coordinate with the medical team. The role requires assessing patient needs, understanding the care plan, and supporting families throughout the planning and transition process.

You will collaborate with clinicians to develop psychosocial interventions, help arrange community resources, and advocate for patients to improve outcomes and reduce readmissions.

Qualifications

• Master’s degree in social work or equivalent required.
• Licensed/Certified as Clinical Social Worker (LCSW), APSW, CISW, or CSW.
• BLS certification required or within acceptable timeframe.

Responsibilities

• Identifies patients with complex care needs and creates a care plan.
• Collaborates with the health care team using strong oral and written communication.
• Develops psychosocial interventions and connects patients to community resources.
• Advocates for patients and families and coordinates discharge planning.
• Arranges resources for medications, housing, transportation, and other needs.
• Supports efforts to reduce hospital readmissions and ensure smooth transitions.

Education

Master’s degree in social work

Licensed/Certified as Clinical Social Worker (LCSW), APSW, CISW, or CSW

American Heart Association Healthcare Provider Basic Life Support (BLS)
