Job summary

Providence Milwaukie Hospital seeks a Social Worker MSW to join Social Work Services on call during the day shift. The CSWA provides biopsychosocial assessments, coordinates care with an interdisciplinary team, and advocates to reduce health disparities in patient care.

Required: MSW and Oregon CSW Associate License within 60 days of hire. Preferred: two years clinical healthcare experience, hospice or bereavement, with a strong commitment to ethical, person-centered care.

Qualifications

• Master's Degree in Social Work.
• Within 60 days of hire: Oregon Clinical Social Work Associate License.

Responsibilities

• Provide comprehensive biopsychosocial/spiritual assessment, diagnosis and/or treatment/interventions/advocacy for patients and their support systems.
• Collaborate with patients and support systems to develop and implement evidence-informed, person-centered care plans across the care continuum.
• Work with an IDT (Inter/Multi-Disciplinary Team) to deliver holistic care and uphold NASW ethics and SOP standards.
