Job summary

Providence Willamette Falls Medical Center seeks a Licensed Clinical Social Worker (LCSW) for social work services on-call during the day. You will perform biopsychosocial assessments, diagnoses, and interventions, collaborating with an IDT to create person-centered care plans across the care continuum.

Qualifications

• Master's Degree in Social Work from a CSWE-accredited program.
• Oregon Clinical Social Worker License or Washington Clinical Independent Social Worker License.
• License in states wherever care is delivered.
• 2 years relevant Social Work experience.

Responsibilities

• Provide biopsychosocial/spiritual assessments, diagnoses and treatment/interventions.
• Collaborate with the patient and support system to develop care plans.
• Work with Inter/Multi-Disciplinary Team (IDT) to implement plans.
• Uphold NASW SOP and mission, promoting equity and reducing disparities.
