Job summary

Providence Willamette Falls Medical Center seeks a Social Worker MSW to join Social Work Services. This full-time day shift position (Tuesday-Saturday, 8:00 a.m. – 4:30 p.m.) involves comprehensive biopsychosocial assessments and care planning under direct supervision.

The CSWA will collaborate with patients and care teams to develop evidence-informed, person-centered plans while upholding NASW ethics and striving to reduce health disparities.

Qualifications

• Master's Degree in Social Worker.
• Within 60 days of hire Oregon Clinical Social Work Associate License.

Responsibilities

• Provide comprehensive biopsychosocial/spiritual assessment, diagnosis and/or treatment/interventions/advocacy of patients and their support systems.
• Collaborate with patient and support systems to develop and implement evidence-informed person-centered care plans with the IDT.
