Job summary

Providence Milwaukie Hospital seeks an MSW Social Worker for on-call, day shift, to provide biopsychosocial/spiritual assessment, diagnosis and intervention under supervision. You will collaborate with the IDT to create evidence-informed, person-centered care plans across the care continuum, guided by NASW ethics, reducing disparities and promoting patient self-determination.

Qualifications

• Master's Degree in Social Work required.
• Within 60 days of hire: Oregon Clinical Social Work Associate License required.

Responsibilities

• Provide comprehensive biopsychosocial/spiritual assessment, diagnosis and/or treatment/interventions/advocacy for patients and their support systems.
• Collaborate with patient and support systems to develop and implement evidence-informed, person-centered care plans across the care continuum with the Inter/Multi-Disciplinary Team (IDT).
• Respect patient self-determination and work to reduce health disparities and barriers to care in alignment with NASW ethics and SOP.

Education
