Job summary

The role includes screening and assessments, care and transition planning, counseling, discharge planning, and resource coordination, with MSW and LCSW-C required; salary range $34.62–$57.19 per hour and a $5,000 sign‑on bonus.

Qualifications

• MSW degree required.
• Valid LCSW-C licensure required.
• Experience in healthcare or mental health settings preferred.

Responsibilities

• Conduct psychosocial assessments for patients.
• Develop care and transition plans.
• Provide direct psychosocial support and coordinate care.
• Identify and address barriers to discharge and coordinate resources.
• Facilitate family meetings and participate in interdisciplinary rounds.
• Maintain timely, accurate documentation per standards.
• Connect patients and families with community resources.
• Provide coverage, on-call, and home visits.
• May provide clinical supervision if approved by the Maryland Board of Social Work Examiners.
• Participate in quality improvement activities and committees as assigned.
• Performs other duties as assigned.
