Job summary

Qualifications

Responsibilities

• Conduct psychosocial assessments for ED patients and families.
• Provide crisis intervention and counseling in a high-volume ED.
• Perform safety assessments and respond to behavioral health crises.
• Develop patient-centered discharge plans.
• Coordinate referrals to community resources and services.
• Collaborate with clinicians and care teams to optimize outcomes.
• Document in EMR per hospital and regulatory standards.
• Mentor social work students and interns as assigned.
