Job summary

Johns Hopkins Howard County Medical Center in Columbia, MD is seeking a Community Social Worker (CSW) to join the Hospital’s Community Care Team. The CSW will conduct psychosocial assessments, provide education, advocacy, counseling, and support, and help clients connect to behavioral health services.

The role maintains a caseload and collaborates with CHN and CHW to determine appropriate care. The CSW supervises the CHW in the Emergency Department and helps coordinate care with medical

Qualifications

• Graduate of an accredited college or university; MSW required.
• Two or more years of experience in public health, behavioral health, or a related field preferred.
• Experience in acute care and/or community settings preferred.

Responsibilities

• Conduct comprehensive psychosocial assessments.
• Complete home visits as needed.
• Provide education, advocacy, counseling, and support.
• Manage assigned caseload.
• Assist clients with behavioral health treatment and community resource connections.
• Support CHNs and CHWs in assessing psychosocial needs and determining appropriate care.
• Coordinate with medical providers and care team members to support comprehensive care.
• Complete accurate and timely documentation.
• Supervise the CHW embedded in the Emergency Department.
