Job summary

UCF Lake Nona Hospital is seeking a Social Worker (MSW) to provide biopsychosocial assessments and discharge planning for inpatients and ED patients in a compassionate, patient-centered manner.

You will collaborate with a multidisciplinary team to identify needs, address barriers to discharge, and coordinate care transitions while supporting patients and families in difficult decisions and emotional challenges.

Qualifications

• Masters Degree in Social Work required.
• Ability to perform biopsychosocial assessments and interventions.

Responsibilities

• Collaborate with RN case managers, nurses, physicians, insurers, and staff to identify patient needs.
• Identify barriers to discharge and recovery (financial, emotional, psychosocial, medical, spiritual).
• Maintain knowledge of Medicare, Medicaid, and private payer regulations and managed-care concepts.
• Perform thorough assessments and develop discharge plans for high-risk patients with psychosocial needs.
• Coordinate meetings between care team, patients, families, and caregivers for difficult decisions.
• Provide psychosocial support and referrals for emotional, financial, and resource needs.
• Offer emotional support during end-of-life, crisis situations, and complex family communications.
