Job summary

LewisGale Medical Center in Salem, VA is seeking a Social Worker (MSW) to perform non-clinical biopsychosocial assessments and interventions on inpatient floors and the outpatient ED. The role includes crisis intervention, discharge planning, and referrals to financial assistance or community resources.

You will work with care teams to identify barriers to discharge, support patients and families, and coordinate psychosocial services, with a focus on end-of-life discussions and emotional support

Qualifications

• Masters Degree required or equivalent professional experience in social work.
• 1 year of experience in social work or related field.

Responsibilities

• Collaborate with RN case managers, nurses, physicians, insurance companies and staff to identify patient needs.
• Identify barriers to discharge and recovery, including financial, emotional, psychosocial, medical and spiritual factors.
• Maintain knowledge of Medicare, Medicaid and private payer regulations and managed-care concepts.
• Perform thorough assessments and develop discharge plans for high-risk patients.
• Coordinate meetings between care team, patients and families/caregivers.
• Intervene for psychosocial support, counseling, advocacy, and information referrals.
