Job summary

LewisGale Medical Center in Salem, VA seeks a mental health professional to create discharge plans addressing patients’ psychological, social, cultural, and financial needs. You act as a mediator among clinical teams, patients, families, and next-level care providers to ensure seamless care.

Responsibilities include leading psychoeducational groups, contributing to treatment plans, safety assessments, and post-discharge follow-up, while coordinating with the MDT for patient readiness and safety.

Qualifications

• Requires Bachelor's degree.
• BCLS certification required or must be obtained within 30 days.
• Nonviolent Crisis Intervention certification within 60 days.
• Minimum 1 year of experience.

Responsibilities

• Conducts individual, group, and family psychoeducational groups.
• Contributes to treatment plan development and attends treatment planning meetings and reviews on an ongoing basis.
• Assesses environment and patient’s safety status and reports/responds to actual and potential safety issues or changes in the patient's condition.
• Identifies risk factors, screening criteria, and interventions for patients with special precautions (e.g., suicide, homicide, falls, elopement).
• Completes relevant assessments, group notes and all other documentation promptly and in compliance with requirements, as appropriate to scope.
• Collaborates with the multidisciplinary team to assess and coordinate patients’ discharge planning needs and readiness for discharge.
• Conducts post-discharge follow-up as indicated.
