Haven Hospice in Florida seeks a compassionate MSW/Social Worker to join our hospice team in Palatka and Gainesville. You will assess psychosocial needs, provide counseling, coordinate resources, and support families through end-of-life care.

Strong communication, organization, and driving to patient homes are essential for holistic care. The role offers daytime hours, office-based placement with field visits, and a stable team environment within Haven Hospice, part of our broader network.

Qualifications

• Master's degree in social work (MSW) accredited by CSWE or a CSWE-accredited Bachelor's in Social Work.
• Minimum of one year healthcare experience; hospice experience preferred.
• Current licensure to practice in the state of operation (Medicare guidelines).

Responsibilities

• Assess psychosocial needs of patients and families.
• Evaluate home safety; assist with alternative care plans.
• Provide counseling, crisis intervention, and emotional support.
• Maintain accurate and timely clinical documentation.
• Educate and assist with advance directives.
• Provide referrals and practical support for patients and families.
• Serve as liaison with community resources and agencies.
• Collaborate with team members to support holistic patient care.
• Support funeral, financial, and legal planning with families.

Haven Hospice is seeking a passionate, dedicated Social Worker to join our team inPalatka, FL. Our Social Workers provide emotional support, counseling, and resource coordination to patients facing terminal illness. This role plays a key part in the delivery of holistic care and ensures that patients' psychosocial needs are met with dignity and respect

Qualifications

• A graduate of a master's program in social work (MSW) accredited by the Council on Social Work Education OR a graduate of a bachelor's program in social work accredited by the Council on Social Work Education. Meets personnel qualifications stated in *418.114(b)(3).
• Minimum of one (1) years' experience in health care, hospice experience preferred. Understands hospice philosophy, and issues of death/dying.
• Experience in a hospice care preferred.
• Demonstrates good verbal and written communication, and organization skills.
• Current licensure according to Medicare guidelines to practice in state of operation.
• Must be a licensed driver with an automobile that is insured in accordance with state or organization requirements and is in good working order.
