Haven Hospice is seeking a dedicated MSW / Social Worker to join our hospice team in Palatka, FL. The position supports terminally ill patients and families with counseling, resource coordination, and psychosocial care.

This full-time role offers collaboration with a multidisciplinary team, a patient-centered approach, and opportunities to grow skills in grief counseling and emotional support across Palatka and surrounding areas.

Qualifications

• Master of Social Work or Bachelor of Social Work with CSWE accreditation.
• Strong verbal and written communication skills.
• Ability to assess psychosocial needs and coordinate care.

Responsabilités

• Assess psychosocial needs of patients and families.
• Evaluate home safety and assist with care plans.
• Provide counseling and crisis intervention.
• Maintain accurate clinical documentation.
• Educate on advance directives and resources.
• Coordinate referrals with community resources.

Haven Hospice is seeking a passionate, dedicated Social Worker to join our team inPalatka, FL. Our Social Workers provide emotional support, counseling, and resource coordination to patients facing terminal illness. This role plays a key part in the delivery of holistic care and ensures that patients' psychosocial needs are met with dignity and respect

As a Social Worker You will:

• Assess psychosocial needs of patients and families
• Evaluate home safety; assist with alternative care plans when needed
• Provide counseling, crisis intervention, and emotional support
• Maintain accurate and timely clinical documentation

• Provide referrals and practical support for patients and families
• Serve as liaison with community resources and agencies
• Collaborate with team members to support holistic patient care
• Support funeral, financial, and legal planning with families

Qualifications

• A graduate of a master's program in social work (MSW) accredited by the Council on Social Work Education OR a graduate of a bachelor's program in social work accredited by the Council on Social Work Education. Meets personnel qualifications stated in *418.114(b)(3).
• Minimum of one (1) years' experience in health care, hospice experience preferred. Understands hospice philosophy, and issues of death/dying.
• Experience in a hospice care preferred.
• Demonstrates good verbal and written communication, and organization skills.
• Current licensure according to Medicare guidelines to practice in state of operation.
• Must be a licensed driver with an automobile that is insured in accordance with state or organization requirements and is in good working order.
