Job summary

Haven Hospice in Palatka, FL is seeking a Hospice Admissions RN to join our team. You will visit patients at home, hospitals and facilities to assess eligibility and coordinate the start of hospice services, delivering patient-centered care.

The role offers 9-6 or 10-7 schedules with alternating weekends, strong team support, and opportunities for growth within a growing hospice organization.

Qualifications

• Graduate of an accredited nursing school with current licensure to practice in state of operation.
• One year of hospice nursing experience required; 3+ years preferred or similar setting considered in place of hospice experience.
• Must be a licensed driver with insured vehicle in good working order.

Responsibilities

• Visit patients (homes, hospitals, nursing facilities) referred to hospice care.
• Explain hospice services, discuss goals and care with patients and families, obtain consents and documents.
• Obtain medical history, current medications, and symptoms.
• Perform comprehensive clinical assessments to determine eligibility for hospice per Medicare/insurance guidelines.
• Coordinate start of services (nursing, aides, equipment, medications).
• Collaborate with interdisciplinary team and maintain timely documentation.

As a Hospice Admissions RN You will:

• Visit patients (in homes, hospitals, nursing facilities, etc.) who are referred for hospice care

• Obtain medical history, current medications, and symptoms
• Perform comprehensive clinical assessments to determine eligibility for hospice based on Medicare/insurance guidelines
• Coordinate the start of services (nursing, aides, equipment, medications.)
• Collaborate with interdisciplinary team
• Maintain accurate and timely documentation

Qualifications

• Graduate of an accredited nursing school with current licensure to practice in state of operation
• One year of hospice nursing experience required, 3+ years preferred or similar setting such as Long-Term Care, Intensive care, or Emergency room will be considered in place of hospice nursing experience
• Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order
