Job summary

Haven Hospice in DeLand, FL is seeking a dedicated Registered Nurse Case Manager to provide patient-centered hospice care in home and facility settings. Schedule: M-F 8-5, with on-call rotation as required by the local branch.

You will assess needs, create care plans, administer medications, manage symptoms, educate families, and collaborate with an interdisciplinary team while maintaining thorough documentation. Must hold a FL RN license and have nursing experience; hospice experience preferred.

Qualifications

• Graduate of an accredited nursing school with current licensure to practice in state of operation.
• One year nursing experience required, 3+ years preferred.
• Experience in hospice or a similar setting preferred.
• Knowledge of the hospice philosophy of care.
• Commitment to clinical and documentation excellence.
• Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order.

Responsibilities

• Assess/monitor physical, emotional, and psychological needs of patients.
• Create hospice care plans that align with the patient's wishes and goals.
• Direct nursing care: administering medications, treatments, and interventions.
• Provide pain and symptom management.
