Job summary

Brio Living Services in Jackson, MI is hiring a Social Worker (MSW) for Thome PACE. This full-time exempt position includes a 40-hour week, Monday–Friday, 8:00 am–4:30 pm, with regular home visits.

You will provide compassionate, person-centered case management, counseling, advocacy, and support as part of the Interdisciplinary Team. Responsibilities include assessing participants, developing care plans, coordinating 24-hour care when needed, and connecting families to community resources, with

Qualifications

• Master's degree in Social Work from an accredited School of Social Work.
• Current licensure as a Social Worker in the State of Michigan.
• Minimum of two (2) years of experience in a health-related field, particularly with the elderly or frail population, desired.
• CPR certification or willingness to obtain within 90 days of employment.
• Valid driver's license & reliable transportation.
• Proficiency in Microsoft Office Suite (Word, Excel, Outlook, PowerPoint).
• Strong communication skills, both verbal and written, with the ability to interact effectively with participants, families, and interdisciplinary team members.
• Ability to multi-task in a fast-paced environment while managing multiple priorities effectively.
• Experience with Electronic Health Records (EHR) and documentation in a clinical setting, desired.

Responsibilities

• Provide compassionate, person-centered case management, counseling, advocacy, and support for older adults.
• Collaborate with the Interdisciplinary Team to assess participants and develop individualized care plans.
• Coordinate home visits and monitor 24-hour care delivery.
• Refer participants and families to community resources and services.
• Document participant changes and progress in the EHR in a timely manner.
• Assist with enrollment/disenrollment processes and Medicaid-related tasks.
