Job summary

Brio Living Services is seeking a Per Diem Social Worker (MSW) in Ypsilanti, MI. The role involves clinical case management, counseling, and resource coordination for older adults in the community, under the Social Work Manager.

Requires an LMSW/LLMSW license in Michigan, CPR certification, and 2 years in related health care with elderly experience. Per diem schedule with flexible hours; compensation $58,000–$62,000 per year.

Qualifications

• Must have LMSW or LLMSW license in Michigan.
• Minimum of 2 years in health-related field with at least 1 year serving frail/elderly.
• Requires reliable transportation and valid driver's license.
• CPR certification required.
• Ability to lift and move up to 35 pounds.

Responsibilities

• Participate in interdisciplinary team assessments and care planning (at least every 6 months).
• Maintain written case management records and reassess participants.
• Coordinate 24-hour care delivery and provide individual/family counseling.
• Refer to community agencies and advocate for participants.
• Assist with Advance Directives and guardianship documentation.
