Qualifications

• Master's Degree in Social Work (MSW) required.
• Current Michigan Social Work licensure required.
• Home health or healthcare experience preferred.

Responsibilities

• Assess patients' social, emotional, financial, and environmental needs.
• Provide counseling, support, and guidance to patients and their families.
• Connect patients with community resources and support services.
• Assist with discharge planning and long-term care planning.
• Collaborate with interdisciplinary home health team.
• Advocate for patients and help address barriers to care.
• Complete documentation in accordance with agency requirements.
