Job summary

Qualifications

• Master's degree in Social Work from CSWE-accredited program.
• Current MSW licensure issued by the South Carolina Board of Social Work Examiners is required at time of hire.
• Preferred: Certification in Case Management.

Responsibilities

• Assist with coordination of the discharge process with the health care team.
• Interviews patients and relatives to obtain social history related to medical problems.
• Identify risk factors after discharge and apply discharge interventions to reduce them.
• Bridge discharge to outpatient providers to address issues and questions.
