Job summary

MUSC Community Physicians (MCP) is seeking a Social Worker to support discharge planning and care coordination at Lexington County Detention Center. You will collaborate with the health care team to ensure a smooth transition and positive patient outcomes.

The role includes assessing patients and caregivers, identifying discharge risks, and connecting patients with outpatient services. A Master’s in Social Work and SC licensure are required; case management certification is preferred.

Qualifications

• Master's degree in social work or social welfare program accredited by CSWE.
• MSW licensure by SC Board of Social Work Examiners.
• Certification in case management preferred.

Responsibilities

• Assists with coordination of the discharge process with the health care team.
• Completes patient and caregiver assessments during hospitalization and follow-up as needed.
• Identifies post-discharge risk factors and applies interventions to reduce risk.
• Bridges discharge to outpatient provider and addresses questions.
