Job summary

The Medical University of South Carolina is seeking a Social Worker I (MSW) to support discharge planning and care coordination across inpatient and outpatient settings in Charleston. Under supervision, you will assess patients and caregivers, identify discharge risks, and develop interventions with the health care team to improve outcomes.

Education: Master’s degree in Social Work from CSWE-accredited program; Licensure: current and active MSW licensure in SC required at hire.

Qualifications

• Master's degree in Social Work from CSWE-accredited program.
• Current and active MSW licensure issued by the South Carolina Board of Social Work Examiners is required at the time of hire.
• Certification in Case Management is preferred.

Responsibilities

• Interviews patients and relatives to obtain social history relevant to medical problems and planning.
• Assists patients with environmental difficulties that interfere with obtaining maximum benefits from medical care.
• Serves as liaison between medical and nursing staffs, patients, relatives and appropriate outside agencies.
• Interprets and assists in resolving social problems that relate to medical condition and/or hospitalization.
