Qualifications

• Master's degree in Social Work (MSW) from CSWE-accredited program.
• CSW or RSW certifications required; LMSW preferred with LMSW exam within 6 months.
• Two years medical setting experience preferred.
• Strong data entry and computer skills; proficiency with Windows apps.

Responsibilities

• Conducts patient assessment interviews, provides referrals and coordinates discharge plans.
• Identifies and provides helpful resources to patients and families.
• Maintains patient records and communicates critical information to the medical team.
• Maintains professional and technical knowledge through continuing education and development.
• Participates in on-call rotation.
