Job summary

Ochsner Health is seeking a qualified social worker to conduct bio-psychosocial assessments, coordinate discharge plans, and connect patients and families with resources. The role involves collaboration with the multidisciplinary team to address emotional, social, and cultural needs while maintaining and updating medical records.

Required MSW from CSWE-accredited program, CSW or RSW certification, and willingness to obtain LMSW within 6 months.

Qualifications

• Master's degree in Social Work from CSWE-accredited university.
• CSW or RSW certification; LMSW preferred with plan to obtain LMSW within 6 months.
• Two years of medical setting experience preferred.
• Must be able to travel between facilities.
• Proficiency with computer systems and data entry.

Responsibilities

• Conduct patient assessment interviews, provide referrals and coordinate discharge plans.
• Identifies and provides helpful resources to patients and families.
• Maintains patient records and documents and communicates critical information to the medical team.
• Maintains professional and technical knowledge and required certifications through continuing education and development.
• Utilizes management resources.
• Participates in on-call rotation.
• Adapts behavior to the specific patient population, including but not limited to: respect for privacy, method of introduction to the patient, adapting explanation of services or procedures to be performed, requesting permissions and communication style.
