Job summary

CHI St. Vincent Hot Springs is seeking a compassionate Social Worker to advocate for individuals and families facing complex challenges. You will coordinate care with an interdisciplinary team and guide patients through available resources and discharge planning.

Responsibilities include assessing needs, educating families and staff, resolving concerns, and communicating effectively with medical teams. A sign-on bonus up to $20,000 is offered for eligible hires.

Qualifications

• Master of Social Work (MSW) degree or higher.
• MSW licensure in Arkansas (AR) or eligibility.
• Basic Life Support (CPR) certification within 30 days.

Responsibilities

• Assesses and develops a comprehensive care plan with an interdisciplinary team.
• Educates patients, families and staff on resources and access.
• Resolves issues and directs concerns to appropriate personnel.
• Demonstrates tact, professionalism and effective communication with patients and staff.
• Provides feedback and coordination for discharge planning.
• Communicates patient progress with physicians and hospital staff.
