Job summary

St. Charles Health System is seeking a licensed clinical social worker or licensed professional counselor to join our care team. The role involves collaboration with the interdisciplinary care team to plan and review care, advocate for patients, and coordinate services for discharge planning.

You will provide bio-psychosocial, trauma-informed interventions, assess risks, and deliver education and referrals to support patients and families across the continuum of care.

Qualifications

• Master's degree in Social Work or Behavioral Sciences required.
• Oregon licensure as LCSW or LPC required.
• Oregon driver's license and SCHS driving eligibility required.
• ACMA/ NASW membership preferred.

Responsibilities

• Collaborate with the Interdisciplinary Care Team to plan and review patient care.
• Advocate for patients’ needs and coordinate services for discharge planning.
• Provide bio-psychosocial and trauma-informed interventions.
• Assess risk and provide brief interventions for emotional distress.
• Maintain accurate records and comply with privacy and legal requirements.
