Job summary

HCA Houston Healthcare Northwest is seeking a Social Worker (BSW) to promote patient-centered care by evaluating psychosocial needs and coordinating post-acute resources. You will liaise with the care team to implement discharge plans and facilitate patient movement across the continuum of care, primarily on Med/Surg units.

The role requires a Bachelors and a Masters in Social Work, with preferred licensure (LCSW/LMSW/RCSW-I).

Qualifications

• Bachelor's degree is required.
• Master's degree is required.
• Licensed social worker (LCSW) or LMSW/RCSW-I preferred.

Responsibilities

• Work with the care team to identify and address patient needs.
• Address discharge barriers across financial, emotional, psychosocial, medical, and spiritual aspects.
• Assess high-risk patients and develop discharge plans.
• Coordinate input from RN case managers to ensure alignment with care plans.
• Participate in interdisciplinary care planning and proactive discharge planning.
• Educate patients and caregivers about community resources and post-acute care criteria.
