Universal Health Services is seeking a dedicated Social Worker for our Partial Hospitalization Program in Newark, DE. The full-time role focuses on psychosocial assessments, discharge planning, and coordinating community resources within a behavioral health hospital setting.

You will collaborate with the interdisciplinary team to address psychosocial and environmental factors impacting recovery, with an emphasis on family involvement and aftercare planning.

Qualifications

• Master's Degree in Social Work; license eligible.
• LMSW required.
• Previous experience in behavioral health setting preferred.
• Current CPR Certification.

Responsibilities

• Conduct psychosocial assessments with patient and family to document history and identify treatment focus.
• Participate in development and implementation of treatment plans with concise, realistic care plans.
• Provide individual, group and family therapies and other didactic groups using multiple modalities.
• Document treatment in medical records legibly and timely.
• Develop and coordinate an individualized discharge plan with the treatment team.
• Identify community resources to meet aftercare needs.
• Attend treatment team meetings to present cases and coordinate care with the interdisciplinary team.
