Job summary

Rockford Center in Newark, DE, a part of Universal Health Services, seeks a full-time Social Worker to support adults in a Partial Hospitalization Program. The Social Worker will collaborate with an interdisciplinary team to address psychosocial and environmental factors affecting treatment.

Qualifications

• Master's Degree in Social Work from an accredited college (must be license eligible).
• LMSW required.
• Previous experience in behavioral health setting preferred.
• Current CPR Certification.

Responsibilities

• Conduct psychosocial assessments to document patient history and identify treatment focus.
• Participate in development and implementation of treatment plans with concise care plans.
• Provide individual, group and family therapy using various modalities.
• Document treatment provided in medical records in a timely manner.
• Develop and coordinate an individualized discharge plan with the treatment team.
