Job summary

Rockford Center in Newark, Delaware, a 138-bed behavioral health hospital, seeks a Social Worker to support patients and families across the care continuum. You will conduct psychosocial assessments, coordinate individualized discharge plans, connect patients with community resources, and advocate for services that promote recovery and long-term well-being, working closely with an interdisciplinary team.

Qualified candidates have a Master’s in Social Work and license eligibility; CPR

Qualifications

• Master's Degree in Social Work from an accredited college; license eligible.
• LPCMH/LCSW preferred.
• Previous experience in behavioral health setting preferred.
• Current CPR Certification.

Responsibilities

• Conduct psychosocial assessments to document patient history and identify issues for treatment.
• Develop and implement treatment plans in coordination with the treatment team.
• Provide individual, group, and family therapies using appropriate modalities.
• Document treatment in the medical record in a timely manner.
• Develop discharge plans and coordinate aftercare needs.
• Identify community resources to support aftercare needs.
• Attend treatment team meetings to coordinate each case with the interdisciplinary team.

Rockford Center is a 138-bed mental health facility located in Newark, DE, providing easy access to a full complement of inpatient and partial behavioral health programs for children and adolescents, adults, and older adults. Rockford is the only child inpatient behavioral health facility in DE, offers an experienced and tenured senior management team, and serves New Castle County and northern Maryland. Position Summary

The Social Worker plays a vital role in supporting patients and their families throughout the continuum of care within a behavioral health hospital setting. This position is responsible for conducting psychosocial assessments, developing and coordinating individualized discharge plans, connecting patients with community resources, and advocating for services that support recovery and long-term well-being. The Social Worker collaborates closely with the interdisciplinary treatment team to address psychosocial, emotional, and environmental factors that may impact treatment outcomes.

Responsibilites

• Conduct psychosocial assessments to document patient history and identify preliminary issues for treatment focus by interviewing the patient and their family members.
• Participate in the development and implementation of treatment plans, ensuring that concise, realistic care plans are carried out.
• Provide individual, group and family process-oriented therapies as well as conduct other didactic groups using various professional treatment modalities.
• Document treatment provided to patient in medical record legibly and in a timely manner.
• Develop and coordinate an individualized discharge plan for the patient by utilizing the treatment team and chart information to determine aftercare needs.
• Identify and assess family or community resources such as group homes, or mental health practitioners to meet patient's aftercare needs.
• Conduct and attend regular treatment team meetings to provide social work perspective to total case management of the patient with the interdisciplinary team by presenting patient’s case and progress towards treatment plan goals, making treatment recommendations, and actively coordinating each case with all members of the treatment team.
