Qualifications

• Master’s degree in social work or social welfare program accredited by the CSWE
• One year post graduate clinical social work experience; three years preferred
• LMSW/LISW licensure by SC Board; LISW-CP preferred.

Responsibilities

• Coordinate discharge process with health care team to ensure smooth transition.
• Conduct psychosocial assessments of patients and caregivers during hospitalization.
• Arrange outpatient follow-up as needed; provide direct clinical social work services in acute psychiatric settings.
• Bridge discharge to outpatient providers to address issues post-discharge and questions.
