Job summary

InnovAge is seeking a licensed Master’s level social worker to provide psychosocial assessments, care planning, and case management for participants in California. The role includes in-home visits, discharge planning, and strong collaboration with the IDT and family members.

Applicants should have at least one year in a healthcare setting, CPR/BLS and First Aid valid in CA, and reliable personal transportation.

Qualifications

• Master’s degree in social work from a CSWE-accredited program.
• CPR/BLS and First Aid required in California only.
• Require personal transportation, current state driver’s license, and auto insurance as required by law.
• One year of health care experience; experience conducting psychosocial assessments, care planning and case management.

Responsibilities

• Conducts initial psychosocial assessments and reassessments.
• Completes in-person visits and coordinates discharge planning and transitions.
• Serves as primary liaison between participant/family and the IDT; coordinates admissions and transportation.
• Documents housing encounters and collaborates with care teams; supports end-of-life coordination and comfort care.
• Maintains accurate documentation across systems and supports Medicaid eligibility processes.
• Participates in IDT meetings and department training; provides community education and inter-agency coordination.
