Job summary

Alternate Solutions Health Network seeks a part-time Medical Social Worker (MSW) to provide access to community resources and ensure medical, emotional, and safety needs are met for patients under home health services. You will follow state rules and participate in care planning, documentation, and coordination with physicians and staff.

You will assess patient needs, connect them to resources, support families, and assist with financial aid applications while maintaining compliance and quality

Qualifications

• One year of Medical Social Work experience required.
• Ability to create positive impressions and communicate with a variety of people.
• Maintain effective communication with patients, families, physicians and co-workers.
• Ability to remain calm, have patience and be accommodating.
• Compassionate and caring while working with patients.
• Knowledgeable on social work best practices.
• Ability to make appropriate judgments.
• Ability to identify a situation and handle it with the best possible solution. Detail-oriented and observant.
• Disciplined style of work ethic. Ability to prioritize and be timely.

Responsibilities

• Conduits assessment of patient needs and identifies barriers.
• Participates in the development of the plan of care.
• Observes and monitors patient conditions.
• Informs relevant staff of changes in the client's condition when necessary.
• Educates team members on social and emotional factors related to health problems.
• Assists patients and families to understand and follow medical recommendations and utilize home health services.
• Connects patients and families to community resources and support.
• Assists with patient financial aid applications.
• Counsels patient and family related to coping and grief.
• Documents all aspects of care and education.
• Maintains active social work license in state of employment.
• Drives to patient locations and completes documentation within 24 hours.
