Job summary

Enhabit Home Health & Hospice is seeking a qualified social worker to support patients needing intermittent professional services in Wyoming. You will educate families and care teams, assess needs, and coordinate with the physician and care plan to ensure quality home-based care.

The candidate must hold a CSWE-accredited MSW, be licensed in the state, and have at least one year of health-care social work experience. Strong communication and technology skills are essential.

Qualifications

• Master's degree in social work from a CSWE-accredited program.
• Current state social work license.
• At least 1 year of social work experience in a health care setting.
• Basic technology skills including use of a mobile device.

Responsibilities

• Administer social services to patients and educate the patient, family, and care team.
• Deliver services per physician orders and the plan of care (POC).
