Job summary

Enhabit Home Health & Hospice seeks a licensed MSW to administer social services and coordinate care with patients, families, and the health care team, following physician orders and the plan of care.

This role requires a master's degree in social work, state licensure, and at least one year of health care social work experience; strong technology skills including mobile devices are essential.

Qualifications

• Must have a masters degree from a school of social work accredited by the council on social work education.
• Must be certified or licensed in the state where they currently practice.
• Must have at least one year of social work experience in a health care setting.
• Must have basic demonstrated technology skills, including operation of a mobile device.
• Medicare home health or hospice experience is preferred.

Responsibilities

• Administer social services to patients requiring intermittent professional services and teach the patient, family, and other members of the health care team.
• Perform services in accordance with the physician’s orders and the established plan of care (POC).
