Job summary

Enhabit Home Health & Hospice seeks a licensed Master’s level social worker to provide patient-centered social services in a home health or hospice setting. You will assess needs, coordinate care with the health care team, and teach patients and families within the established POC and physician orders.

The role requires an MSW, state licensure, at least one year of health care social work experience, and comfort with mobile technology to document and communicate care plans.

Qualifications

• Must have a masters degree from a school of social work accredited by the council on social work education.
• Must be certified or licensed in the state where they currently practice.
• Must have at least one year of social work experience in a health care setting.
• Must have basic demonstrated technology skills, including operation of a mobile device.

Responsibilities

• Administer social services to patients requiring intermittent professional services and teach the patient, family, and other members of the health care team.
• Perform services in accordance with the physician’s orders and the established plan of care (POC).
