Job summary

Haven Hospice in DeLand, FL is seeking a compassionate Social Worker to join our team. You will provide emotional support, counseling, and resource coordination to patients facing terminal illness, ensuring psychosocial needs are met with dignity.

The role involves assessments, crisis intervention, education on advance directives, and collaboration with the care team to support holistic patient care. Licensure, MSW/BSW, and hospice experience preferred.

Qualifications

• Master’s degree in Social Work (MSW) accredited by CSWE or a Bachelor’s in Social Work (BSW) accredited by CSWE.
• Minimum of 1 year experience in health care; hospice experience preferred.
• Current licensure to practice in the state of operation; valid driver’s license with vehicle in good working order.

Responsibilities

• Assess psychosocial needs of patients and families.
• Evaluate home safety; assist with alternative care plans when needed.
• Provide counseling, crisis intervention, and emotional support.
• Maintain accurate and timely clinical documentation.
• Educate and assist with advance directives.
• Provide referrals and practical support for patients and families.
• Serve as liaison with community resources and agencies.
• Collaborate with team members to support holistic patient care.
• Support funeral, financial, and legal planning with families.
