Amedisys in North Carolina is seeking a Masters-level Hospice Social Worker to support patients and families at home. This part-time role covers Alamance/Orange/Chatham/Rockingham/Caswell and includes mileage reimbursement.

Qualifications

• Holds a master’s or doctorate degree from a CSWE-accredited school.
• One year of social work experience in a health care setting.
• Meets state and federal requirements for this position.
• Current CPR certification.

Responsibilities

• Assists patient and family with health-related financial, social and emotional concerns and helps with coping.
• Provides social work services per the plan of care.
• Ensures changes in clinical status are communicated to the health care team and care center staff.
• Educates patient and family on social work interventions and plan of care.
• Performs care planning, transfer and discharge functions.
• Identifies risks in the patient’s environment and implements interventions to reduce risks.
