Qualifications

• Master's Degree in Social Work required.
• Minimum five years of clinical social work experience.
• Valid and current LCSW or LCSW license required.

Responsibilities

• Provide 1:1 home visits to patients and families.
• Assist clients in obtaining government funds, education, or treatment.
• Develop and implement viable care plans and psycho-social assessments.
• Guide crisis counseling and support for individuals and families.
• Coordinate plans to restore patient social and health functioning.

Education

Master's Degree in Social Work

Five+ years of clinical social work experience

Licensed Clinical Social Worker or Licensed Medical Social Worker license
