Job summary

Qualifications

• Holds a master's or doctorate degree from a CSWE-accredited school of social work.
• One year of social work experience in a health care setting.
• Meets state and federal requirements for this position.
• Current CPR certification.

Responsibilities

• Assists patient and family with health-related financial, social and emotional concerns and coping skills.
• Provides social work services in accordance with the plan of care.
• Communicates changes in the patient's clinical status to the health care team promptly.
• Educates patient, family and team on social work interventions and treatment plan.
• Performs care planning, transfer and discharge functions.
• Identifies risks in the patient’s environment and implements interventions.
