Qualifications

• Master's degree in social work from a CSWE-accredited program.
• Current state license to practice social work.
• At least 1 year of social work experience in a healthcare setting.
• Basic technology skills, including operating a mobile device.

Responsibilities

• Administer social services to patients requiring intermittent professional services.
• Teach patients, families, and health care team about the plan of care (POC).

Education

Master's degree in Social Work (CSWE-accredited)

State licensure to practice social work

1+ year social work experience in healthcare

Basic technology skills, including mobile devices

Medicare home health or hospice experience preferred

Illinois MSW preferred for Illinois licensure paths
