Job summary

The Elliot Health System’s Care Coordination Department seeks a Social Worker Case Manager to deliver clinical social work across neonates to geriatrics, coordinating care and resources to maximize outcomes and cost effectiveness.

You will collaborate with the multidisciplinary team, develop discharge plans, monitor progress, and coordinate referrals. A Master of Social Work is required; NH LICSW preferred, with 1–3 years in a hospital setting.

Qualifications

• Master of Science in Social Work required; NH LICSW preferred.
• 1–3 years of experience in a health care system, preferably in a hospital setting.
• Excellent clinical documentation, and successful usage of EMR systems preferred.

Responsibilities

• Collaborates with the primary nurse and multidisciplinary team in ongoing clinical social work assessment.
• Assist in development, implementation, and evaluation of integrated clinical pathways.
• Monitors patient progress to ensure care is appropriate and timely.
• Develops discharge plan in collaboration with the interdisciplinary team.
• Coordinates referrals.
• Utilization review.
