Job summary

Southern New Hampshire Health is seeking a Social Worker Case Manager for the Inpatient Behavioral Health unit. This role provides clinical interventions addressing patient and family needs, coordinates care across disciplines, and facilitates discharge planning to optimize patient outcomes.

The position requires an MSW level education and strong EMR documentation. Experience in hospital-based behavioral health and collaboration with care teams is highly valued.

Qualifications

• MSW degree required; NH LICSW preferred.
• 1–3 years of healthcare experience, preferably hospital.
• Excellent clinical documentation; EMR experience preferred.

Responsibilities

• Collaborate with the multidisciplinary team for ongoing social work assessment.
• Develop, implement and evaluate integrated clinical pathways.
• Provide supportive counseling and crisis intervention for patients and families.
• Monitor patient progress to ensure timely and appropriate care.
• Coordinate discharge planning and family meetings; advocate for patient outcomes.
