Job summary

Enhabit Home Health & Hospice in Florida is seeking a dedicated Social Worker to administer patient social services under physician orders and the care plan. You will educate patients and families while collaborating with the health care team to support home-based care.

The role requires an MSW, active state certification/licensure, and at least one year in a health care setting. A valid driver’s license and CPR certification are required; prior home health experience is preferred.

Qualifications

• Must possess a master’s degree in social work (CSWE-accredited).
• Must be certified or licensed in the state where they practice.
• Must have at least one year of social work experience in a health care setting.
• Must have basic demonstrated technology skills, including operation of a mobile device.

Responsibilities

• Administer social services to patients requiring intermittent professional services and teach the patient, family, and other health care team members.
• Perform services in accordance with the physician’s orders and plan of care (POC).
