Job summary

Novant Health is seeking a Case Manager with an MSW to join our Case Management team at Thomasville Medical Center. This part-time role (24 hours) involves three 8-hour shifts, including weekends, with some holidays.

The Case Manager leads psychosocial assessments, discharge planning, and care coordination within our hospital. Education includes a Master’s Degree in Social Work (MSW) and licensure requirements.

Qualifications

• MSW required and graduate degree.
• Two+ years of professional social work or healthcare experience preferred.
• Licensure/certification: state-approved curricula for restrictive interventions within 3 months of hire.

Responsibilities

• Lead assessment and discharge planning for complex cases.
• Collaborate with physicians and healthcare team to implement proactive care plans.
• Coordinate care and resources for psychosocially complex patients.
• Support discharge planning and care coordination across post-acute options.
